University of Lethbridge Minor Participant Forms

Thank you for allowing your child to participate in our research project.  The following two forms are to be completed by a parent or guardian on behalf of the participant prior to participating in activity with LTRA (can be a week prior). This can be done at any time either at home or immediately before the monitored activities at LTRA.
